What Caused an Infection After a Wisdom Tooth Broke During Extraction?

I have an infection after a tooth broke off during a wisdom tooth extraction. Will I need another procedure to remove the portion my dentist left behind? Or is treating the infection enough? It took over a week for my dentist to admit that the tooth broke as she was removing it. Thanks. HK from Texas

After a straightforward wisdom tooth extraction with an open socket, infection is uncommon because fluids can drain naturally. Infections occur more often when an impacted tooth requires stitches that close the gum over the site.

If a full root or large fragment remains in the socket, the gums can heal over it and trap bacteria inside the jaw. This situation may lead to persistent pain, swelling, or infection.

How to Diagnose the Source of Your Symptoms

Continuing symptoms deserve evaluation from another dentist, ideally an oral surgeon who can both diagnose and treat the problem. An X-ray provides the clearest way to see whether any tooth structure remains.

This process helps determine:

  • Whether a retained root is causing symptoms
  • Whether the bone has healed over the area
  • How complex the removal would be

If a significant root was left behind, requesting a refund could be a reasonable next step.

Understanding Root Fractures During Extraction

Tooth roots sometimes break during removal, particularly in older adults whose roots may thicken over time. This complication can happen even with careful treatment and does not necessarily reflect poor care.

Appropriate management depends on the fragment size:

  • Small tips in sensitive areas may remain without causing harm
  • Larger pieces typically require removal by the dentist or an oral surgeon

Patients should always receive a clear explanation when this occurs.

Do I Still Have an Infection Months After Tooth Extraction?

After seeing my dentist for a broken crown, she took an x-ray of the tooth and said it was infected. My dentist referred me to an endodontist for the root canal. Months after the root canal, I felt referred pain in the tooth in front of the root canal tooth. I reported the issue to my dentist during my next cleaning appointment. An x-ray showed the tooth was still infected.

I called the endodontist, and she prescribed penicillin. I’ve had a series of family health issues and could not return to the endodontist as life would have it. Two months ago, an oral surgeon extracted the tooth. Still, I have jaw pain below the area of the tooth extraction. I was planning to get a dental implant to replace the extracted tooth, but I am concerned that something is not healing correctly.

Unfortunately, you could not return to the endodontist to save your tooth. Still, you are wise to pursue getting a dental implant. An implant will preserve your jawbone in the space and prevent other teeth from shifting, disturbing your bite, and creating orthodontic issues.

Although it is rare to have a lingering bone infection after tooth extraction, it can occur. If you have an infection, a potent antibiotic can get rid of it. We recommend that you return to the oral surgeon for an examination.

Also, a post-operative infection can result from an antibiotic-resistant infection. So, you would not want to retake penicillin to get rid of the infection. Ask the oral surgeon to try different antibiotics to help you progressively heal.
