My tooth is turning dark. My dentist says I need a root canal, but I feel no discomfort. Does a dark tooth always need a root canal? I can’t fit it into my budget. Thanks. Vance from SoCal

Although a darkening tooth may not always need a root canal, an exam and X-ray can reveal its condition and whether it is infected.

A dark tooth often indicates a dead or dying tooth pulp when the living tissue inside the tooth becomes infected and decays. While you may not feel pain if the tooth is dead, the infection can spread and cause serious health problems.

Why You Shouldn’t Delay Root Caal Treatment

The Journal of Endodontics published results from a study that showed 66 people died from tooth infections from 2000-2008. Additionally, the Sacramento Bee and Fox News reported the case of Californian Vadim Kondratyuk Anatoliyevich, a 26-year-old husband and father of two girls. Vidim was a long-haul truck driver who developed a tooth infection. In January 2017, while driving to New York on a long-haul job, Vadim’s tooth infection worsened, so he stopped in Oklahoma to see a dentist who prescribed antibiotics. As Vadim continued his travel to New York, the toothache got better, then worse, and the side of his face began to swell.

Vadim made his delivery in New York but could not drive home due to the severity of his tooth infection. Vadim’s brother flew to New York to drive him back to California. However, Vadim had difficulty breathing and became pale, so his brother took him to a Utah hospital, where he was placed on oxygen and flown to a Salt Lake City facility.

Root canal treatment gets rid of a tooth infection

The infection had spread to Vadim’s blood and lungs. Despite stronger antibiotics and dialysis, the doctors in Salt Lake City could not save him. Vadim’s wife was able to fly to the hospital in Utah to say goodbye before he died.

At-home care is not enough to treat a tooth infection. Antibiotics are insufficient because an infected tooth is dead and does not have blood circulation to transport medication. If you have a toothache, address it by seeing a dentist for root canal treatment—the only way to remove infection from a tooth. Can I Get a Root Canal If I Have a Porcelain Veneer?

My dentist says I have a periapical abscess and I will need a root canal on one of my front teeth. However, my front teeth all have veneers over them. Will this case discoloration? Or will I need my veneers replaced?

Hi Brent,

Usually, when porcelain veneers are placed, they are done on teeth that have been prepared. Most preparation cuts ridges into the surface of the tooth. However, some dentists go too deeply, or even place crowns under the guise of  veneers. Doing either of these things will cause significant stress on the surrounding teeth. The stress could be the cause behind the root canal.

Getting a root canal on a tooth will cause the tooth to turn dark, and this can lead to discoloration. If the tooth beneath the veneer is discolored, it can show through the veneer. To avoid discoloration, your dentist will need to clean your tooth properly, and may need to replace the post with white fiberglass and use natural-looking composite to fill in any gaps. An expert cosmetic dentist should be able to help you avoid dental discoloration, but if you trust your general dentist, they can help you as well.

Can I Trust This Dentist?

Hello,

I went to see a new dentist about some tooth pain I’ve had for a few weeks. This is the first time I’ve felt pain in this tooth, so I didn’t think it was too serious. However, the dentist told me one of my teeth has a cavity and needs an extraction. He would have to put me under to do either a bone graft or dental implant. Should I get a second opinion? I don’t want to get all this work done over some tooth pain. He didn’t even find an infection, but gave me antibiotics for the pain. I’m mostly worried about having a reaction to the anesthetic. I’m terrified of getting dental work done and am dreading this. What should I do?

Hi Claire,

If your tooth has recently started feeling pain, it has only become infected recently. It likely isn’t so far gone that it needs extractions, let alone a bone graft or implant. A tooth that’s beyond saving would have been hurting for awhile, and would have a visible cavity in it.

A second opinion would be a good idea. You can bring any X-rays to your new dentist. A fresh set of eyes can give you a new perspective on your issue.

Another point to consider is that anesthetic is a safe medicine, and you are not going to have an allergic reaction to it. Dental anxiety is nothing unusual. Find a second opinion from a sedation dentist. They are experienced in administering nitrous oxide or local anesthetic for anxious patients. The more anxiety you feel, the harder it will be to get you numb. Too much anxiety, and you’ll need conscious oral sedation. A gentle dentist will be able to advise you on your sedation needs.
