Will Invisalign Correct Crowding in My Mid-40s?

I have a couple of quick questions related to Invisalign. I’m in my mid-40s and had my wisdom teeth removed when I was 17. When I had the teeth extracted, they told me I wouldn’t need braces because removing my wisdom teeth would give space for my other teeth. Over the years, my teeth have become somewhat crowded, and it’s becoming obvious I will have to do something to fix it if I ever want to feel comfortable about smiling. Could I have done something to cause this since they told me I would not need braces? Is this something Invisalign can solve? Thanks. Danielle

 

Dear Danielle,

Thank you for your question. Dr. Brooksher would need to examine and x-ray your teeth before recommending treatment. But orthodontic treatment can correct teeth crowding.

What Causes Teeth Crowding?

Genetics, teeth grinding, tooth loss, or oral habits during childhood can cause teeth crowding. Your tooth position can change. That’s why clear braces or traditional braces can help.

Clear aligner trays similar to Invisalign and ClearCorrectReasons for overcrowding of teeth may include the following:

  • Genetics: Some people have naturally smaller jaws or have teeth that are too wide for their jaws. Others have narrow palates and other oral conditions that can cause tooth malalignment. When teeth do not come together naturally, the constant biting force can push them out of place.
  • Grinding: Regularly applied force of any kind, including teeth grinding, can shift teeth. Grinding, particularly at night, is a big culprit for adults.
  • Extended Pacifier/ Bottle Use: Sucking on things—even thumbs—creates forward-moving pressure. Although most adults will not have an issue from prolonged bottle and pacifier use, some continue similar behaviors in adulthood, such as chewing on pens or thrusting their tongue against their front teeth.
  • Tooth Loss: If a tooth is missing, the other teeth will begin to close the gap, the way water will rush into a hole carved on a sandy beach. It can gradually occur. Tooth loss also impacts the position of the tooth that would normally bite against the missing tooth. For example, if you lose a premolar on your lower left, you’ll likely have shifting on your lower and upper left teeth.
  • Oral Health: If you do not maintain regular cleaning appointments and good care at home, you will develop periodontal disease. In the early stage, it can cause bleeding gums, but when it progresses to an advanced stage, you will likely have bone loss in your jaw and lose teeth.
  • Poor Dental Work: When dental repairs are not properly completed (this occurs more often with dental tourism), poorly shaped crowns, fillings, bridges, and other work may change the bite or begin to push teeth out of alignment.
  • Injuries: Car accidents, falls, and other forms of trauma can impact your teeth in many ways.
  • Tumors: Tumors and other medical conditions can impact your jaw, teeth, and bite, although rare.
  • Failure to Wear a Retainer: Many orthodontists recommend wearing a retainer at night for life or having a permanent fixed retainer attached to the back of your teeth. Failure to follow the orthodontist’s advice after braces will likely result in shifting.

Clear Braces Can Correct Teeth Crowding

In most cases, clear braces are an excellent choice to solve crowding and can address some underlying issues as well. You will need a dentist to evaluate the degree of crowding, but you must people have success with clear aligners. Schedule a consultation.

I am unhappy with my denture, so I spent the past few weeks researching my options. I am interested in overdentures but would like to know which ones are more affordable. Is it worth going with the least expensive type? I’ve worn complete dentures since 2002, and most people think I am 10 years older than my age. Which Implant Overdentures Are the Most Affordable?

The most affordable implant overdentures are removable and require at least two dental implants to retain your denture. A snap-on denture is a popular option that improves your experience with dentures.

What Is a Snap-On Denture?

A snap-on denture is a removable implant denture. The denture base has two to four grooves that snap onto dental implants. Although you can get a snap-on denture with just two implants, more implants will increase denture stability.

Snap-On Denture Advantages

You will notice these benefits from your implant-retained denture:

  • Feels more natural
  • Makes it easier to eat and speak with your denture
  • Stimulates your jawbone to prevent further shrinkage

A snap-on denture will outlast and outperform a complete removable denture. A June 2017 article in the Journal of Prosthodontics reviewed 25 studies that compared removable full dentures with implant-retained dentures. Studies show that implant-retained dentures significantly improve chewing function and quality of life.

Snap-On Denture Disadvantages

The disadvantages below compare a snap-on denture to a fixed implant denture:

It does not fit, feel, or function as well as a non-removable denture

  • . You must remove the denture to clean it.
  • You must remove the denture overnight while you sleep.

How Much Is a Snap-On Denture?

The average cost of one snap-on denture (upper or lower) with two dental implants in the U.S. is $10,000 – $15,000, but each patient’s case is unique. Your cost will depend on your needs, which may include several factors, such as these:

  • Your dentist’s fees for x-rays and a 3-D CT scan, which is essential for accurate implant placement
  • Bone grafting procedure
  • Bone grafting materials
  • Dental implant surgery
  • Dental implant parts
  • Wear temporary dentures while your implants heal
  • Custom denture
  • Follow-up visits

Also, costs vary depending on where you live in the U.S., how many implants you receive, and the denture you choose.

Fixed Implant Overdentures

I am a 37-year-old adult, but my teeth are small and look like baby teeth. I feel like only my gums show when I smile, and the tiny teeth beneath them are almost invisible. It is embarrassing. My friends try to comfort me by saying it’s better than having teeth that are too large, but I need to feel confident and smile more. How Can You Make Small Adult Teeth Larger?

You can make small adult teeth larger with the help of a cosmetic dentist. Two treatment options include dental bonding and porcelain veneers. We will compare how the options can transform your smile.

How Do Dental Bonding and Porcelain Veneers Enlarge Small Teeth?

Dental bonding and porcelain veneers enlarge small teeth by using cosmetic dentistry to expand their appearance. The treatment options have different procedures, materials, longevity, and costs. Let us explain.

Dental bonding

Below we explain dental bonding composition, procedure, longevity, maintenance, and the average cost for enlarging small adult teeth.

  • Composition. Dental bonding is a blend of plastic resin and silica filler. Bonding material has properties like tooth dentin (the layer beneath the enamel). A cosmetic dentist can polish bonding to a natural-looking gloss.
  • Procedure. After lightly etching your tooth enamel, a skilled cosmetic dentist blends composite and layers it by hand to achieve your desired tooth shape and size.
  • Longevity. Dental bonding is less durable than porcelain and has a shorter lifespan. High-quality bonding can last five years or longer if you care for it well and maintain it with a cosmetic dentist’s help. A study published in a 2011 Japanese Dental Science Review issue concluded that at least 60 percent of high-quality dental bonding could last up to ten years.
  • Maintenance. Floss between your teeth and use a soft-bristled brush and nonabrasive toothpaste. Dental bonding is not stain resistant, but a cosmetic dentist can polish it for you.
  • Cost. Chairside bonding costs $300 to $1,000 per tooth. The price depends on the condition of your teeth, the extent of work, the dentist’s skills and fees, and your location in the U.S.

Porcelain veneers

Porcelain veneers composition, procedure, longevity, maintenance, and cost differ from dental bonding in several ways, which we will explain.

  • Composition. A dental ceramist can craft porcelain to mimic the color and characteristics of natural teeth while making your small teeth look more prominent. Porcelain veneers contain dental ceramics and are more durable than tooth enamel.
  • Procedure. Your cosmetic dentist may recommend conservatively preparing your teeth for porcelain veneers by moving tiny amounts of enamel from their fronts. After taking impressions of your teeth, your cosmetic dentist will make temporary veneers to determine whether they require adjustments. You will get custom veneers after you approve the temporary ones.
  • Longevity. Well-made and precisely bonded porcelain veneers can last about 20 years. A 2021 European Journal of Dentistry article published findings from a cumulative, long-term study on the lifespan of porcelain veneers in patients with good oral health and oral hygiene habits. The results after five, ten, fifteen, and 20 years are below:
    • Five years: 98%
    • Ten years: 96%
    • Fifteen years: 91%
    • Twenty years: 91%
  • Maintenance. Schedule dental cleanings and exams with a trained cosmetic dentist. You can floss and brush your veneers as your natural teeth but avoid abrasive substances.
  • Cost. One porcelain veneer can cost $1,500 to $3,500 depending on tooth condition, personalization, veneer quality, and your dentist’s fees.
