In March 2024 my former dentist completed four dental implants and a new denture. My dentist said I had enough bone for the implants, so I did not need a bone graft. Last December, I noticed movement in the overdenture I had felt before. I saw the dentist once about the issue, and without giving too many details, the dentist was evasive. I left the office because I was angry and knew I could not play the game with my dentist. Now, I feel soreness in my gums around two implants. I am terrified of dental implant failure. Besides that, I have dental anxiety. Then there is the thought of contacting my dentist again for a refund. How to Find a New Implant Dentist

You can find a new implant dentist by checking websites to see if they have post-graduate implantology training. Look for credentials from the International Congress of Oral Implantology or the American Academy of Implant Dentistry.

My Implant Dentures Messed Up My Bite

It is possible to have an allergic reaction to your acrylic denture base. Titanium allergies are rare, but it is possible to react to dental implants. You would need testing to confirm whether you are allergic to denture materials. However, stressful dental visits are sometimes associated with burning mouth syndrome.

Your denture has disrupted your bite.  A dentist must correct it to prevent symptoms of jaw muscle spasms, headaches, and other symptoms of TMJ disorder. We recommend consulting a dentist with advanced training in occlusion and bite. The dentist can ensure upper and lower denture teeth meet correctly when you close your mouth or chew.

Will My Snap-On Denture Be Too Small?

I am concerned that my snap-on denture arch will be too small, not going back far enough in my mouth. My new denture doesn’t fit right. Can I get it lengthened, or should I get my dentist to fix it before it’s too late? I found a dentist whose presentation convinced me I would get good-quality affordable implant dentures because he would do the work in phases. I would get a denture first and implants within 6-10 months or as soon as I could afford them. It sounded good to me, but the new denture is too small. Won’t my snap-on denture be too small? With the new denture, I feel my upper and lower gums touching when I chew. When I open my mouth and look in the mirror, I can tell that the denture stops short of where my wisdom teeth were. Although a removable denture will not feel perfectly comfortable, a dentist can ensure it extends far enough to prevent you from chewing on your gums. If your new denture is too small and your dentist’s plan is to fit it with implants, your snap-on denture will not fit correctly.

How Far Back Should a Denture Extend?

A denture should extend just beyond the retromolar pad, your wisdom teeth’s former location. Accurate impressions of your mouth help a dentist and dental lab produce a denture that fits well without covering too much or too little of the retromolar pad. A denture that extends too far back upper and lower denture teeth to clash. However, when the denture arch is too short, you can feel chewing on your gums.

It will require more work from him and the dental lab. You can see from the photos on this page that the denture impression tray extends farther than the impression tray for teeth, registering the retromolar area and broader parts of the lower jaw. Ensuring that a denture extends back far enough improves denture fit and stability.

We cannot speculate on the details of your case, including how your dentist took impressions of your mouth or if he consciously decided to shorten the arch length. Still, you have the right to be satisfied with your current denture before planning to secure it with dental implants. Otherwise, getting affordable implant dentures can cost you a lot of time and money in the long term.

Is It too Late for Implant Dentures?

Even if you have worn removable dentures for many years, it is still possible to get implant dentures. Although you have experienced bone shrinkage, which naturally occurs in areas of missing teeth, a skilled implant dentist may recommend ridge augmentation first.

What Is Ridge Augmentation?

Ridge augmentation is a surgical procedure to replace missing bone. Your implant dentist or oral surgeon will add bone grafting material in areas of bone shrinkage and cover it with a protective material. According to the 2020 article, Ridge Augmentation in Implant Dentistry, you can expect the following healing time after a bone graft:

  • Autogenous (your own) bone: Four to six months
  • Non-autogenous (donor or synthetic) bone: At least twelve months

The most affordable implant overdentures are removable and require at least two dental implants to retain your denture. A snap-on denture is a popular option that improves your experience with dentures.

What Is a Snap-On Denture?

A snap-on denture is a removable implant denture. The denture base has two to four grooves that snap onto dental implants. Although you can get a snap-on denture with just two implants, more implants will increase denture stability.

Snap-On Denture Advantages

You will notice these benefits from your implant-retained denture:

  • Feels more natural
  • Makes it easier to eat and speak with your denture
  • Stimulates your jawbone to prevent further shrinkage

A snap-on denture will outlast and outperform a complete removable denture. A June 2017 article in the Journal of Prosthodontics reviewed 25 studies that compared removable full dentures with implant-retained dentures. Studies show that implant-retained dentures significantly improve chewing function and quality of life.

Can a Dentist Make My Implant Denture Teeth Smaller?

The front teeth in my implant denture are too big. Although I didn’t take care of my teeth most of my life, I never had buck teeth, but I do now with my implant dentures. I talked to the dentist about it, but he said that for the low price, which was 2/3 of what most dentists charge, some aspects of the denture might not be exactly what I wanted. I knew his price was lower, but the dentist never mentioned that I would have buck teeth. It doesn’t make sense to me, anyway. Why make the teeth too large when I didn’t request it? My Implant Denture Makes My Lips Look Bulky

Should permanent dentures (all on four) be placed with the anterior teeth 2 to 3 mm from the gum line, or should they follow the gum line? Is this a standard procedure or the dentist’s choice? I have bulky lips from a denture that protrudes. I’m doing some research and figuring out what to do next and whether I can trust my dentist. – Thanks, Demetrius from NJ

Before we answer your question, we want to define the gumline. As shown in the picture to the right, the gumline is the edge where the gum meets a tooth. Consider some examples:

  • If a tooth is extracted, there is no longer a gumline.
  • If a tooth is replaced with a dental implant, the gumline is the edge that meets the dental crown.
  • If all your teeth are missing and you receive a complete denture, the gumline is the edge of the pink, gum-colored acrylic that meets the false tooth.

We defined the gumline because it is impossible to place a dental implant at that location.

You’re probably referring to where a dentist places teeth near the jawbone ridge beneath the denture.

Complete removable denture

When you receive a complete removable denture, your dentist should place the denture’s front lower teeth directly over the bony ridge. If the denture is too far forward, it will tilt back when you bite anything with your front teeth.

All-on-4 implant denture

An implant-retained denture, such as All-on-4, is secure and won’t tilt when you bite. The placement of denture teeth 2-3 millimeters from the ridge, as you described, is close enough within range for a removable denture or an implant denture.

Aesthetics and facial appearance are essential for natural-looking results. But measurements alone should not determine where a dentist places your denture. Advanced cosmetic dentists make a wax model of the denture for you to try in before the lab completes the acrylic version. The wax model lets your dentist evaluate how the implant denture affects your facial features, lip, speech, and the overall function of your mouth.

Adjusting your denture to soften the appearance of your lips

If you already have your implant denture, your dentist can remove it and reset the teeth. If the problem persists and your dentist can’t resolve it, you can schedule a consultation with an advanced cosmetic dentist for recommendations. Your dentist may charge a fee to reset the teeth, but if you’ve already expressed satisfaction, they might do it without cost.

I get a migraine before dental appointments

Before every dental appointment, I get a migraine headache. I know this is anxiety, but I can’t stop myself from getting headaches. Of course, I can’t drive myself to the dentist either when I have a migraine. This has been going on for 2 years, during which I have only seen a dentist once.  Last week, I saw a dentist for an urgent appointment because my denture broke. The dentist warned me to fix it promptly to avoid an emergency. My anxiety level is over the top, and I cannot think clearly enough to know if I can trust this dentist for a new implant denture or if I need to look elsewhere. Where do I start? Thank you. Aaron – We suggest that you find a dentist who caters to anxious patients. Find two or three dentists with whom you can schedule a consultation. But ensure the dentists are also skilled with dental implants.

Schedule the consultations without mentioning the kind of dental treatment you need. Explain your anxiety and listen to and watch how each dentist responds to determine which one might be best for your needs.

Affordable dental implants will secure your dentures

Ask each dentist what they do to help patients relax. If sedation dentistry is available, find out what levels of sedation the dentists offer and what you can expect from each level. If oral conscious sedation is an option, you will be able to take a small amount of anti-anxiety medication before your appointment.

Tell the dentist about your migraines. Depending on the sedation type, you may need to arrange transportation to and from your appointments.  And when you receive implant dentures, you can expect to be sedated for the surgery.

There are ways to help you maintain good oral health and to get the dental care you need. Finding the right dentist to meet your needs will get you back on track.

Are Dental Implants Permanent?

Engineers designed dental implants to be a permanent form of tooth replacement—lasting a lifetime. The article, What Is the Success Rate of Dental Implants?, published by the American Academy of Implant Dentistry, confirms that dental implants can last many decades if you take care of them. However, the permanency of dental implants refers to the screw or artificial tooth root. The prosthesis, or artificial teeth, attached to implants will require replacing over the years.

What Are the Parts of a Dental Implant?

Dental implants are artificial tooth roots. Your implant dentist will attach a connector (abutment) to each implant fixture. And after three to four months of healing, your dentist will attach artificial teeth to the abutments. Meanwhile, you can wear temporary replacement teeth.

What Is the Lifespan of a Dental Implant Restoration?

Dental implant crowns, bridges, and dentures can last longer than restorations not attached to implants. Estimates for their lifespan are below:

  • Implant crown – An implant crown can last up to fifteen years if you take care of it. Gold crowns are sturdy and last longer than ceramic crowns.
  • Implant bridge – An implant bridge can last up to twenty years before you need to replace it.
  • Implant denture – A well-made implant denture can last ten years or longer.

What Can You Do to Help Your Dental Implants Last?

Your dental implants can be permanent if you take precautions. Your choice of an implant dentist and diligent home care make a difference.

Are All-on-4 Implants Really My Only Option?

Last month, I had a consultation with a dentist about stabilizing my dentures. They don’t have to be perfectly stable, but I do not want them sliding around anymore. The dentist recommends all on 4 implants. When he told me the cost, I told him that is way out of my budget, and I will have to explore my options. He quickly said that there are no other options because I’ve worn dentures too long. He said that his rate is among the best I will find. Is it possible that all on 4 could be the only way a patient could qualify for implant dentures? If so, I will forget about getting implants and accept my dentures as they are. You mentioned that you had a consultation with this dentist. Based on your description, it seems that you did not have an exam, CT scan, or any x-ray for the dentist to decide that All-on-4 implants are your only option. And that is puzzling. Be careful with this dentist.

If you have worn dentures for many years, you have some jawbone shrinkage. But an implant dentist can build up the bone with grafting to ensure the implants have plenty of support.

What Type of Implant Dentures Can You Receive?

The American Academy of Implant Dentistry clearly explains the types of dental implants available. But a dentist can define your treatment options only after these steps:

  • Reviewing your medical and dental histories
  • Examining your teeth and gums
  • Reviewing a CT scan

Affordable Implant Dentures

A snap-on denture is an affordable alternative to All-on-4

A snap-on denture or a denture retained with mini-implants is more affordable than the All-on-4 technique. Those affordable options use as few as two dental implants. Although more implants will improve a denture’s stability, your treatment costs will increase.

And not every patient qualifies for All-on-4. We recommend that you look for an implant dentist with extensive post-graduate training and schedule an appointment for an exam. The dentist will explain your treatment options. A conscientious dentist will discuss the pros and cons of each option but will not insist that you receive a specific type of treatment—or any treatment at all.
